چکیده

When antiretroviral therapy (ART) scale up began in Africa a decade ago, there was concern that lack of laboratory monitoring for patients receiving antiretroviral drugs would be an obstacle to providing treatment. To address this concern, the first set of World Health Organization (WHO) guidelines for ART in resource-limited settings rightly recommended that programmes should primarily use clinical criteria for monitoring treatment efficacy [1].

Modelling response to HIV therapy without a genotype: an argument for viral load monitoring in resource-limited settings.

In the absence of widespread access to individualized laboratory monitoring, which forms an integral part of HIV patient management in resource-rich settings, the roll-out of highly active antiretroviral therapy (HAART) in resource-limited settings has adopted a public health approach based on standard HAART protocols and clinical/immunological definitions of therapy failure. The cost-effective...

Effect on transmission of HIV-1 resistance of timing of implementation of viral load monitoring to determine switches from first to second-line antiretroviral regimens in resource-limited settings.

BACKGROUND There is concern that antiretroviral therapy (ART) use with only clinical monitoring for failure will result in high rates of transmission of virus with resistance to drugs currently in use. METHODS A stochastic simulation model of transmission of HIV, natural history and the effect of ART, was developed and used to predict the proportion of new infections with resistance according...

Measurement of viral load by the automated Abbott real-time HIV-1 assay using dried blood spots collected and processed in Malawi and Mozambique.

BACKGROUND The use of dried blood spots (DBS) for HIV-1 viral load quantification can greatly improve access to viral monitoring for HIV-infected patients receiving treatment in resource-limited settings. OBJECTIVES To evaluate and validate HIV viral load measurement from DBS in sub-Saharan Africa, with a reliable, all-automated, standard commercial assay such as the Abbott m2000. METHODS A...

Point of Care Diagnostics for HIV in Resource Limited Settings: An Overview

Human immunodeficiency virus (HIV) is a global health problem. Early diagnosis, rapid antiretroviral therapy (ART) initiation and monitoring of viral load are the key strategies for effective HIV management. Many people in resource limited settings where timely access to medical care is a challenge and healthcare infrastructure is poor have no access to laboratory facilities and diagnosis is de...

Durable Suppression of HIV-1 after Virologic Monitoring-Based Antiretroviral Adherence Counseling in Rakai, Uganda

OBJECTIVES HIV viral load is recommended for monitoring antiretroviral treatment and identifying treatment failure. We assessed the durability of viral suppression after viral load-triggered adherence counseling among patients with HIV viremia 6 months after ART initiation. DESIGN Observational cohort enrolled in an antiretroviral treatment program in rural Uganda. METHODS Participants who ...
